Covenant Advantage (HMO-POS) and University of Michigan Health Advantage Flex (PPO) are plans with a Medicare contract. Enrollment in our plans depend on contract renewal. All Covenant Advantage (HMO-POS) and University of Michigan Health Advantage Flex (PPO) plans include Part D drug coverage.
To enroll in a Covenant Advantage (HMO-POS) and University of Michigan Health Advantage Flex (PPO) plan, you must have both Medicare Parts A and B and reside in the service area. You must continue to pay your Medicare Part B premium. Enrollment in a Covenant Advantage (HMO-POS) and University of Michigan Health Advantage Flex (PPO) plan may be limited to specific times of the year.
Out-of-network/non-contracted providers are under no obligation to treat PHP Medicare members, except in emergency situations. Please call our customer service number or see your Evidence of Coverage for more information, including the cost-sharing that applies to out-of-network services.
Every year, Medicare evaluates plans based on a 5-star rating system.
